CDISC Life Membership is the highest award available to recognise individuals who have provided exemplary service and contribution to the Central Districts Indian Sports Club.

To nominate an eligible CDISC member for Life Membership, please download and complete the Life Membership Policy & Nomination Form here, and email through to the CDISC Secretary by the 31st of December each year.
